Registration

Registered conference delegates including students are entitled to attend all scientific sessions as well as the Friday workshop, and will receive a copy of the Program and the Conference Proceedings. In addition, the Welcoming Reception is included in this registration fee.

Participants who register before 10 May 2011 save money and time onsite with the convenience of a pre-registration arrival process. Furthermore, delegates who choose to stay at the conference venue, the Westin Hotel Ottawa, are eligible for a $100 Facility User Discount.

NEW SESSION ADDED: Roundtable on Screening and Surveillance for HAVS 

This half-day Roundtable will provide an opportunity to share experiences and discuss perspectives on the challenges of developing successful screening and surveillance programs for HAVS.  What does experience show us from the various jurisdictions where programs (of various kinds) have been put in place to address worker injuries of the upper extremities?  What can we learn from this?  The Roundtable will include presentations and panel discussions on what’s not working, and what seems to be working, which in turn will provide direction on how such programs can be improved in the future.

The Roundtable is open to all HAV 2011 delegates.  The registration fee for those wishing to attend only this half-day session is $150.

NOTE 1: Registrants choosing the Facility User Discounted rate (those planning to stay at the Westin) are required to submit their hotel reservation confirmation number during the online registration process.

Registration

Online registration will be open from 1 November 2010 to 9 June 2011. On this date, registration will close and will reopen onsite at 1:00 p.m. on 13 June 2011 in the foyer of the fourth floor of the Westin Hotel, Ottawa.

Official Receipt

An electronic receipt will be sent by email to all PAID delegates. Certificates of Attendance will be available upon request, and delivered onsite.

Cancellation and Refunds

HAV 2011 participants may withdraw their registration in writing prior to 13 May 2011. All refunds are subject to a $75.00 CAD administration fee. No refunds will be accepted after 13 May 2011.
